Your employer or insurance company may ask your doctor to fill out forms about your work ability and readiness to return to work. Your completed job analysis will help your doctor better understand your ability to do your former job or any alternative job your employer proposes. Your job analysis can also help your doctor specify limitations that will need to be accommodated. Based on your doctor’s recommendations, your employer will figure out if a job is ready for you, or if they can make the necessary changes for you to return to work.

Once you have completed your job analysis form, give the summary sheet to all your doctors. You can also give a copy to any other healthcare professionals from whom your employer or insurance company asks for an opinion on your work ability and readiness to return to work.
